Forms marked with an * are located in AHGFamily. All forms will be included in the camp packet.
The swim test must be completed prior to camp, it is not possible to complete one at camp.
The High Adventure Activity Form must be signed by a physician, be sure to make an appointment in advance of camp. High Adventure forms are only valid for 12 months from exam, be sure to check the dates on your form.
